Supporting Documentation

Supporting documents needed for services:

• A current photo ID for each household adult age 18 yrs. or older
• Social security card for everyone in the household (including children)
• Birth Certificates for everyone in the household (including children)
• Proof of residence (utility bill or lease)
• Income for everyone in the household

If earned income, the proof of income will be your most recent consecutive paystubs for 4 weeks or a signed and dated letter on company letterhead from clients employer that specifies the gross pay per pay period and payment schedule.
